Perth and Kinross Health and Social Care Partnership Awarded Platinum Digital Telecare Implementation Award.

Digital Telecare for Scottish Local Government are delighted to confirm that Perth and Kinross Health and Social Care Partnership has been awarded the Platinum Digital Telecare Implementation Award in recognition of the recent progress of their analogue to digital telecare transition project.
To achieve the Platinum Implementation Award, a Telecare Service Provider must have successfully rolled out a live digital telecare service to 100% of service users and is operating successfully without serious issues or call failures for at least 8 weeks.
The Partnership supports approximately 3,600 individuals and associated housing schemes, with the objective of delivering integrated, person-centred care. This approach enables individuals to maintain healthy and independent lives at home, thereby reducing admissions to hospitals or long-term care facilities.
This accomplishment is the result of extensive collaboration and commitment from a wide range of individuals and teams who have worked together to improve outcomes for local communities.
